Custom Forms with Auto Populate

The custom forms are great, I would like them to be able to use either the client's or caregivers information or even contacts information to auto populate. For example, if I am creating a discharge summary form with the custom form I have, instead of having to manually enter the Name, DOB, Address, Phone, etc., it auto populates based on which type of form we are using.

Ability to delete client records after 10 years (or automatically delete)

Standard health records retention laws are to shred files after 10 years, and to delete digital data. Currently, there is only an option to make a profile inactive, but this status can easily be changed so that the file details are visible.  Need a way to delete the record, automatically delete it, or somehow lock it so only super admins can access

Communication and Tasks

It would be highly beneficial to have the ability to select specific recipients under the Communications section when adding comments or notes. For example, being able to share a note exclusively with a specific caregiver and management rather than all users.

Additionally, it would be helpful to have the option to upload photos or documents directly within comments or notes for better documentation and clarity.

Lastly, applying these same functionalities to Tasks would be valuable—allowing users to assign specific tasks to caregivers, management, etc., while also enabling document and photo uploads within tasks. Only those assigned should be able to view and access the tasks.

Referral Source Leads and Specific referral source

We are trying to track both the lead source (ie hospitals, rehabs, care coordinator) and the specific referral source within the lead source (ie Upstate Hospital, Elderwood Rehab, Aging Advocates Care Coordinator). Currently you can do one or the other. We would like to have the ability to add both the lead and referral as outlined above.

Client Signature Forms

It would be much more convenient if you could attach multiple caregiver emails to send a signature form, instead of individually, as there are more than one caregiver scheduled for a client. Thank you!

Care plan shows fields left blank on assessment and makes the care plan unnecessarily busy.

Please make changes to care plan to hide fields that are not used. The assessment form has a very useful function that allows you to have conditional components that only show if the value of the particular question is "yes" For example, Is Seizure support required? if the answer is "yes" the next three sections will display to allow for all details regarding seizure support to be entered. If the answer to this question is "no" the sections regarding details about seizures will stay hidden. This is great! However, this feature loses it's value when looking at the care plan generated by this assessment, as the care plan will show all the empty fields regarding seizure support, even when the answer to the question Is seizure support required is "no".

Alerts

I have been finding myself purchasing different software just to get the things that I need. I would absolutely love to have everything in one place. Alerts are very imperative to running any form of business, especially healthcare. When it comes to alerts and reminders that is very important for our organization. I think that the alerts should be made priority. 

Examples 


1. I have Medicaid clients that are required to have supervisory visits every 30, 60, 90, or 120 days. If there were a recurring reminder in Caresmartz360_Agency when the supervisory visits are due it will save agencies from a lot of citations, and will help us stay organized and on top of our paperwork

2. Reassessments if a client is due for a reassessment the nurse and agency should get an alert that these reassessments are due. One thing that agencies always get cited for no matter what state is missed supervisory visits and missed reassessments. Having these alerts will assist us in maintaining proper care for our clients, and not missing these visits. 

The alerts piece that is missing from CareSmartz could be the most important thing to all agencies these alerts should be via text message, email and notification through the app.


I also know that there will be a cost to this and this could be made an A la carte option for agencies to pay a little extra for this service, and I am willing to do that. There is already a space for this on the client profile under reassessment the date if there were at least three more boxes added there then this will take care of the alerts portion.

Expiration Dates client document

Hello,

It will be helpful for the agency owners that like to keep track of their paperwork and make sure that all of their paperwork is up-to-date to have an expiration date column in the clients documents. If we have an expiration date, then we will know when documents need to be updated. In some states, you have to have documents, updated, yearly, we have no way of knowing when a clients document expire unless we open the actual document. This feature should include an expiration date alert is well. This will assist agencies with keeping track of everything in an organized manner.
