Add an option to add physician information to client's profile

In the meantime,  you can include this information in the Summary Notes if you wish to also share this information with the caregivers. Or in the Office Notes at the bottom of the Main Profile Tab if they should only be visible to the Office Staff. Contacts are also an option.
