Bruce, just to clarify...you have a medical identification number in Canada that is not in the US, and you would like that number to be part of the assessment.
You COULD use the US Medicaid ID field if that would transfer to the assessment.
Yes. That is the Idea. Ours is called a OHIP (Ontario Health Insurance Plan) Number in Ontario. Every citizen in Province is issued a card with a number for health services. I would like the number to show up in our Care Plan.

Medicaid ID place I can use as long as they don't limit the spaces to insert numbers and letters.
