You can find the configuration for the Gender drop-down in Settings>>Client>>DDM Client Management>>Sex
From there, click the green Add button, and add the selections that are relevant to you and your client base.

As with many configurable fields in Caresmartz360, the gender drop-down is customized under the Client module in the Admin Settings, but cascades out to the Caregiver profiles as well.
Those selections are also available if you collect this information on the application form.
