Please add a birthdate field to the caregiver application.
It should be mandatory, with the basic information.

Date of Birth/applicant age information is not currently collected as part of our default application form. If you have additional information you would like to gather, there is a fully customizable Miscellaneous Question section where you can include those questions. Settings>>Caregiver>>Caregiver Applicant Form Settings>>Enable Miscellaneous Questions
