add the date to the printed assessment

To my knowledge this still hasn't been rectified. I enter my own date in the 2nd address line

This was implemented in the release we did this week.  Please review your completed assessments and let me know if you don't see the date

I checked the care plans we did today and it's there.  Thank you so much for adding this.

I just did an update on a care plan and this one  did not print out the care plan completion date.  Is this only for new care plans or reassessments?  Odd because the other few I looked at did have the information on them.
